We want you to have an efficient and easy registration experience. We encourage you to complete the following pre-registration form. In doing so you will save time and be better prepared for your visit to our facility.
Pre-registration is available for expectant mothers who are planning to deliver a baby at Mercy Hospital of Folsom and Diagnostic Imaging patients.
Family Birth Center registration
At your Obstetric (OB) Physician office you may have received a mail-in pre-registration form. Please complete either the mail-in form or this form online. Please note that to effectively process your pre-registration we must receive your completed form at least 30 days prior to your scheduled or expectant delivery date. If you have less than 30 days before the time of your scheduled or expectant delivery, you may pre-register by telephone. Please call our Patient Registration Office at 916.983.7451. The hours of operation are Monday through Friday, 8 a.m. to 5 p.m.
Diagnostic Imaging registration
Please note that to effectively process your pre-registration we must receive your completed form at least 72 hours prior to your expected service date. If you have less than 72 hours before the time of your scheduled service you may pre-register by telephone. Please call our Patient Registration Office at 916.983.7451. The hours of operation are Monday through Friday, 8 a.m. to 5 p.m.
What You Will Need to Pre-Register Online
Before you begin the online pre-registration process, you will need:
- Your insurance identification card
- Expected delivery date
- Obstetric (OB) Physician
- Personal contact information

By completing this form online, you are submitting your personal information to assist in your pre-registration prior to your arrival at our facility. All information you provide is secure to protect your privacy. After you have submitted your online pre-registration form, you will receive a phone call confirming your pre-registration is complete and provide you with information regarding any financial liability for your delivery.
Please note that there will be forms that will require your signature when you arrive on the day of your delivery. Please bring your photo identification and insurance card.
