Please note that to effectively process your pre-registration we must receive your completed form at least 72 hours prior to your expected service date. If you have less than 72 hours before the time of your scheduled service you may pre-register by telephone. Please call our Patient Registration Office at (916) 983-7451. The hours of operation are Monday through Friday, 8 a.m. to 5 p.m.
What You Will Need to Pre-Register Online:
Before you begin the online pre-registration process, you will need:
- Your insurance identification card
- Expected Service Date
- Personal contact information
Additional Information: By completing this form online, you are submitting your personal information to assist in your pre-registration prior to your arrival at our facility. All information you provide is secure to protect your privacy.
After you have submitted your online pre-registration form, you will receive a phone call confirming your pre-registration is complete and provide you with information regarding financial liability for this procedure and/or any authorizations that may be required by your insurance carrier.
Please note that there will be forms that will require your signature when you arrive on the day of your scheduled service. Please bring your photo identification, insurance card and physician’s order.
